Job Description

Reports to: Community Manager

FLSA Status: Non-Exempt. Position is eligible for overtime.

The Resident Services Coordinator will receive general supervision, direction, and guidance from the Community Manager.

Qualifications:

  • Education: High School diploma or equivalent education required.

  • Experience: Previous resident services coordinator experience. Experience level may vary due to the special needs of the property.

  • Skills: The position requires the ability to deal well with people and get them to feel comfortable quickly. Proficiency in MS Office (MS Excel and MS PowerPoint, in particular)

Attendance: Due to the property staffing limitations, it is extremely critical that the individual be able to work their scheduled hours plus any other hours necessary to complete the job.

Physical Requirements:

  • Stand and walk or sit alternatively depending on the specific needs of the day.

  • The constant need to bend/stoop/squat, climb stairs, push or pull, reach above shoulder, file, open/close doors/drawers, grasp/turning, writing/typing

  • Frequent lifting/carrying (paperwork, deliveries, files, miscellaneous) less than 20 lbs.

The Resident Services Coordinator will comply with established policies and authorized approval. Resident Services Coordinator’s responsibilities include, but are not limited to the following:

Responsibilities

Provide general case management, including, intake, and referral services to residents in need of such assistance.

Provide formal case management for a resident when such service is not available through the general community:

  • Evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident’s situation and needs.

Perform initial assessments:

Establish linkages with agencies and service providers in the community. Determine/develop best price for service in personal assistance, homemaker, meals on wheels, transportation, counseling, occasional visiting nurses, preventative health screening, wellness and legal advocacy.

Educate residents on service availability, application procedures and client rights.

Maintain a directory of providers for use by both project staff and residents.

Advocate, organize, problem-solve and provide results for the elderly and/or disabled population.

Monitor the ongoing provision of services from community agencies and keep the case management and provider agency current with the progress of the individual. Manage the provision of supportive services when appropriate.

Assist residents in building informal support networks with other residents, family and friends.

Develop volunteer support programs with service organizations in the community.

Provide training to project residents in the obligations of tenancy or coordinate such training.

Educate staff on the management team on issues related to aging in place and service coordination to help them to better work with and assist the residents.

Work cooperatively with the management team on issues that threaten the ability of the resident to age in place.

Provide priority assistance to residents that are determined to be frail, elderly and/or at risk.

Receive ongoing training in gerontology, psychology, counseling, and drug and alcohol use/abuse, mental health issues affecting the elderly and/or disabled.
