Schedule: Typically Monday - Friday daytime and evening hours, some weekends and holidays as needed. Wage: $19.00 per hour Job Duties and Responsibilities: Communicate with the support team both verbally and in writing Provide assistance and support to individuals served Write outcomes, procedures, and POCs for individuals founded on needs, wants, and dreams. Demonstrate and act as necessary based on knowledge of systems, routines, applicable licensing regulations, and programming. Facilitate access to healthcare services, educate regarding personal health issues, and support/assist with medical recommendations and directives from health care providers. Encourage participation in leisure activities at home and in the community to promote a healthy lifestyle and socially valued goals. Advocate for the benefit of others.
