This position assists with the implementation of daily program activities for behavioral health patients. Provides support for the patients\' emotional and social needs. This position also monitors and identifies anything needed to promote safe patient care. Major Responsibilities: 1. Provides developmentally appropriate supportive interaction with counseling and monitoring of patients in a 1:1 or group setting. 2. Assess changes in patient\'s behavior and notifies the RN. 3. Assists the treatment team and partner with patient in developing and modifying Behavioral Health Plan of Care for patients. 4. Partners with the treatment team to encourage patient compliance with Behavioral Health Plan of Care and participation in activities. 5. Assists RN with admission, transfer, and discharge process. 6. Facilitates educational groups. 7. Participates in family meetings. 8. Documents in EPIC. 9. Demonstrates role accountability by adhering to Gundersen Health System statements, policies, and confidentiality statement. 10. Performs other job-related responsibilities as requested.
