Cecil County Government Nursing Home Options Counselor in Elkton, Maryland

NURSING HOME OPTIONS COUNSELOR is needed for the Department of Community Services- Aging and Disability Services with salary of $20.37 per hour. Position provides options counseling for institutionalized individuals which includes specific information regarding the spectrum of Waiver programs, referrals to a variety of other community-based programs, eligibility requirements, Medicaid application assistance, administration of the Level One Screening tool and referrals to appropriate agencies. Case management services will be provided throughout the transition period. This position works 20 hours per week, and includes PTO, Workers Comp. and FICA benefits only. It is financed, and contractual under the Level One Screen Grant and renewable based upon continued funding.

Position requirements: Bachelors Degree in Gerontology, Social Sciences, Human Services, or a related field from an accredited college or university; One (1) or more years experience in Human Services; Valid Drivers License.

Duties: Receive and review referrals for options counseling for institutionalized individuals from various sources including MDH, nursing facilities, etc.; Provide Money Follows the Person (MFP) Options Counseling for institutionalized individuals; Educate the individuals by providing accurate and complete information about Maryland Medicaid programs including the Medicaid Home and Community Based Services Waivers and the community-based programs; inform the individual and their representative or guardian of eligibility requirements, provide case management support to the individual in weighing their options; Verify where the individual is in the application process with regard to Long Term Care (LTC) Medicaid and what may be needed before a determination can be made; Document the individuals preferences for future community services and housing; Based on an individuals documented preferences for future community services refer them to the appropriate agency for application or provide application assistance if requested; Confirm or verify the basic Waiver eligibility; Assist the individual in the application process by securing the required signatures, assisting in obtaining required supporting documentation, generating required referrals, submitting the application and the MFP consent form to the appropriate agencies; Enter data in the MFP tracking system, generate reports and produce other associated data when needed; Become knowledgeable of all the case management services that the Medicaid Waiver case manager provides; Maintain current knowledge of applicable local, State, and Federal laws, rules, and regulations regarding long-term care; Prepare and submit quarterly reports; Administer the Level One Screen in accordance with the guidance provided in the Maryland Access Point Manual; Provide case management for a small caseload of Medicaid and Senior Care program participants; Perform all other duties as assigned.
