Position Summary: The Diabetic Educator provides Nutrition referral consultations and provides Diabetes Education referral consultations to Axis Health System patients. The Registered Dietitian & Diabetes Educator provides consultations under the scope of the Registered Dietitian licensure, and Diabetes Education under the American Diabetes Association (ADA) certification, accreditation, and curriculums. In collaboration with the integrated health team, provides health, nutrition, and diabetes education for patients (pediatric/adult) to enable them to achieve self-management and overall health. This position participates in individual appointments, joint visits with providers, group sessions, classes, and outreach. This position will also be responsible for the coordination of AHS ADA Diabetes Education certification program requirements, and mentorship of other Diabetes Education staff. This position's primary location of work will be at Durango Integrated Healthcare with periodic/monthly scheduled days at other SW regional Integrated Healthcare clinics, in addition to utilization of telehealth platforms for pati

Our ideal candidate for our Diabetic Educator position will have their Bachelor's or Master's degree in Nutrition/Registered Dietitian (RD); Diabetes CDCES or CDE certification training and have at least 1 Year experience as Registered Dietitian (RD) and minimum 1 year providing diabetes education under supervised ADA program. The successful candidate will have their Registered Dietician license and have CDCES or CDE certification OR actively obtaining hours to take certification exam.
